W dzisiejszym wpisie chciałabym poruszyć bardzo ważny aspekt dotyczący leczenia ortodontycznego dzieci w wieku od dwóch do sześciu lat. Coraz częściej przychodzą do mnie rodzice ze swoimi pociechami skierowani przez przedszkolnego logopedę, ponieważ oprócz wady wymowy diagnozują także nieprawidłowy zgryz.

Powszechnie sądzi się, że niezbędny jest aparat ortodontyczny. Chciałabym uświadomić wszystkich rodziców małych przedszkolaków, że sam aparat ortodontyczny to nie wszystko i właśnie o tym będzie dzisiejszy wpis.

Gdzie leżą przyczyny rozwijającej się wady zgryzu?

Może to brzmi dziwnie, ale dziecko już od początku swojego życia powinno być ukierunkowywane pod kątem prawidłowego kształtowania zgryzu i wzrostu twarzy. Jeżeli dziecko w ciągu dnia ma permanentnie otwartą buzię, oddycha przez usta, chrapie w nocy, ma krótkie wędzidełko podjęzykowe, ssie kciuk – istnieje bardzo duże prawdopodobieństwo problemu ze zgryzem.

Takie dziecko potrzebuje leczenia przyczynowego na wielu płaszczyznach i samo leczenie aparatem ortodontycznym nie rozwiąże problemu. Usuwanie przyczyn, z powodu których nie następuje harmonijny rozwój szczęk i twarzy, jest tu kluczowe. Praca w ten sposób od najmłodszych lat da znacznie lepsze i bardziej trwałe efekty.

Jaka jest rola rodzica?

Chciałabym podkreślić, że to rodzic odgrywa w tym procesie kluczową rolę. To on powinien dbać i pilnować, aby jego pociecha regularnie, czyli codziennie, i właściwie wykonywała zalecenia specjalistów. To rodzice przebywają z dzieckiem na co dzień, nie lekarz czy terapeuta.

W przypadku nawykowego ssania kciuka, przy współpracy z psychologiem i ortodontą, można uporać się z nawykiem, ale niezwykle ważne jest pozytywne wzmacnianie przez mamę lub tatę.

Aby ćwiczenia mioterapeutyczne zalecone przez ortodontę lub logopedę przyniosły efekt, muszą być wykonywane CODZIENNIE. Specjalista uczy dziecko i rodzica, jak poprawnie wykonywać ćwiczenia, ale nie zastąpi opiekunów w codziennej pracy.

Dlatego dbanie o zgryz u dzieci w wieku przedszkolnym to przede wszystkim edukacja i podnoszenie świadomości wśród rodziców, co przekłada się na motywację do pracy z dzieckiem.

Wielopłaszczyznowy problem

Bardzo często spotykam się z sytuacją, gdy logopedzi odsyłają rodziców z małymi dziećmi do ortodonty, zaniepokojeni wadą zgryzu. Rodzice przychodzą do mnie z nadzieją, że przedszkolak dostanie aparat i nagle w cudowny sposób przestanie np. seplenić, a zgryz zacznie się szybko poprawiać.

Niestety nie jest to takie proste. Jeżeli logopeda widzi wadę ortodontyczną, to jest to złożony problem, którego sedno tkwi nie tylko w wadzie zgryzu.

Najpopularniejszą z przyczyn wad zgryzu i wymowy jest stale otwarta buzia u dziecka. Powoduje to, że język zajmuje złą pozycję w jamie ustnej i łuki zębowe nie rozwijają się tak, jak powinny. Jeśli nie rozwijają się, nie są w stanie prawidłowo pomieścić wszystkich zębów. W efekcie rozwija się stłoczenie zębów, co staje się realnym problemem, gdy wyrzynają się zęby stałe, bo po prostu nie mają miejsca.

Niestety prawie 80% 4–5-latków w ciągu dnia ma otwartą buzię i nie oddycha wyłącznie przez nos. Coraz częściej w otoczeniu mojego 6-letniego syna obserwuję dzieci, które nie mają prawidłowej pozycji języka i jamy ustnej. Łatwo to zauważyć, bo ich usta są stale rozchylone, a zmiany w rysach twarzy zaczynają być już widoczne.

Rodzice powinni na ten problem zareagować i pilnować, aby dziecko zamykało usta – oczywiście pod warunkiem, że nos jest drożny. W takiej sytuacji najprawdopodobniej opieka laryngologiczna, logopedyczna, mioterapia i leczenie ortodontyczne będą niezbędne.

Trzeba mieć świadomość, że otwarta buzia może być spowodowana częstymi infekcjami dróg oddechowych. Dziecko w wieku przedszkolnym przebywając w grupie jest bardzo narażone na infekcje i często biega z zatkanym nosem. Jeśli maluch jest bardziej podatny i ciągle ma katar, naturalne jest, że będzie więcej oddychał buzią. Dlatego również ze względu na to, że przewlekające się infekcje górnych dróg oddechowych mogą prowadzić do nawykowego oddychania przez usta, warto dbać o drożność nosa, co może wiązać się np. z pozostawieniem malucha w domu i wzmocnieniem odporności.

Dziecko ze słabą drożnością dróg oddechowych ma często powiększony trzeci migdałek i podczas snu chrapie, co może pogłębiać wadę zgryzu i wymowy. Im bardziej dziecko ma przerośnięty trzeci migdałek, tym większa skłonność do infekcji – i tak błędne koło się zamyka.

Chciałabym podkreślić, że także noworodki czy niemowlaki powinny spać z zamkniętą buzią. To nie jest normalne, że niemowlę śpi mając uchylone usta. Jeżeli zauważymy, że maluszek od urodzenia ma tendencję do leżenia z otwartymi ustami, starajmy się po karmieniu delikatnie je domykać.

W tym miejscu chciałabym wspomnieć, że karmienie piersią będzie sprzyjać nauce prawidłowego toru oddychania, gdyż dziecko musi nauczyć się jednocześnie jeść, połykać i oddychać z zamkniętą buzią.

Inną przyczyną wad zgryzu i wymowy może być nieprawidłowe ułożenie języka w jamie ustnej. Język powinien leżeć na podniebieniu, za górnymi zębami, i wywierać na nie lekki nacisk. Tak to wygląda, gdy osoba ma szeroki górny łuk zębowy, proste zęby i ładnie ukształtowane środkowe piętro twarzy (które jest zbudowane z kości szczęki).

Jeżeli dziecko ma otwartą buzię, język opada w kierunku dna jamy ustnej i utrwala się zła postawa jamy ustnej. Nieprawidłowa pozycja języka skutkuje nieprawidłową funkcją, czyli niepoprawną wymową i połykaniem.

Kluczem do sukcesu nie jest tylko terapia prowadzona raz w tygodniu, ale aktywne ćwiczenia wykonywane CODZIENNIE w domu pod okiem rodziców.

Na czym polega leczenie zespołowe?

Logopedzi bardzo często kierują do ortodonty najmłodsze przedszkolaki, czyli dwu- czy trzyletnie dzieci, ponieważ zauważają u nich rozwijającą się wadę zgryzu. Takie dziecko jest za małe na leczenie aparatem ortodontycznym, jednak powinno być pod kontrolą ortodonty.

Rolą ortodonty jest uświadomienie rodzica o problemach występujących u dziecka i wzmocnienie jego motywacji do pracy z dzieckiem (ćwiczenia, kontrole u innych specjalistów), a także wkroczenie z aktywnym leczeniem wtedy, kiedy dziecko jest gotowe na współpracę w wykonaniu wycisków i w noszeniu aparatu. Zazwyczaj ma to miejsce około czwartego, piątego roku życia.

Leczenie wad zgryzu u małych dzieci powinno być interdyscyplinarne. W pierwszej kolejności należy skupić się na działaniu profilaktycznym i wzmacnianiu prawidłowej pozycji jamy ustnej – to punkt wyjścia do kolejnych działań. Czyli będę przypominać do znudzenia: ćwiczenia korekcyjne wykonywane CODZIENNIE.

Dzięki temu dobrze ułożony język, który jest naturalnym aparatem ortodontycznym u każdego człowieka, będzie wpływał na rozbudowę łuków zębowych u malucha w taki sposób, że wszystkie zęby w przyszłości pomieszczą się, a twarz będzie rosła w kierunku doprzednim (co jest atrakcyjne), zamiast pionowym (nieatrakcyjna, podłużna twarz).

Jednocześnie, jeżeli zachodzi potrzeba (a u większości dzieci tak jest), powinno się rozpocząć rozbudowę łuków zębowych aparatem. W wieku 5–10 lat życia realizuje się około 99% wzrostu twarzoczaszki. Żuchwa rośnie nieco później, niejako dopasowując się do szczęki, która stanowi środkowe piętro twarzy. Jak więc wpływać na doprzedni wzrost szczęki, kiedy po 10. roku życia jest on już ukończony?

Niestety często dzieci z niedorozwojem szczęk są odsyłane na leczenie ortodontyczne dopiero po wyrżnięciu się wszystkich stałych zębów. Zęby w wieku nastoletnim wyprostujemy, na wzrost twarzy niestety mamy wówczas znikomy wpływ.

Podsumowując: leczenie przedszkolaków powinno być zespołowe i skupiać się na rozwiązywaniu problemów na różnych polach. Niezmiernie ważna jest świadomość i motywacja rodziców, gdyż to od nich zależy, jak pokierują rozwojem dziecka.

Orthodontic treatment in pre-school children

In today’s post I would like to raise a very important aspect regarding orthodontic treatment in children aged two to six. More and more parents are coming with their children for consultation at my surgery directed by a kindergarden language therapist who happened to diagnose not only speech defects but also a malocclusion. It is generally believed that an orthodontic appliance is necessary in such case. I would like to argue that an orthodontic appliance by no means can fix everything and that’s exactly what today’s entry will be about.

What are the reasons of the developing malocclusion?

Maybe it will sound a bit strange, but every child should be directed towards the correct formation of bite and facial growth from day one. If a child keeps its mouth permanently open throughout the day, breathes through it, snores at night, has a short sublingual frenulum, sucks its thumb, there is a very high probability of potential bite problems in the future.

Such a child needs causal treatment on many levels and a sole treatment with an orthodontic appliance will not solve the problem. Eliminating the causes which hinder a harmonious development of the jaws and face is crucial. Working this way from an early age will give much better and more lasting results.

What is the role of a parent?

I would like to emphasize that a parent can play a key role in this process. This is because they ought to make sure that their child regularly, and that is every day, carries out the recommendations of a doctor or a specialist in a proper way. After all, it is parents who takes care of and monitor their child on a daily basis, not a doctor or a therapist. In the case of habitual sucking of the thumb, it is possible to deal with the habit in a joint cooperation with a psychologist and an orthodontist, but a positive reinforcement by the mother or father is extremely important.

Therapeutic exercises recommended by an orthodontist or speech therapist will only bring effects if performed EVERY DAY.

Of course, it is a role of a specialist to teach a child and the parents how to properly perform the exercises, but that cannot replace the caregivers in the daily „orthodontic supervision” of their child. That is why fixing the bite of pre-school children primarily boils down to education and raising awareness among parents, which should result in good motivation to work with a child every day.

A multi-faceted problem

I often encounter a situation when speech therapists alarmed with the potential malocclusion refer parents with small children to an orthodontist. Lots of parents simply hope that their preschooler will get an appliance and suddenly, in a magic way, their child will stop lisping for instance and the bite will instantly improve. Unfortunately, it is not so easy. If a speech therapist detects an orthodontic defect, it is an increasingly complex problem, the essence of which lies not only in the malocclusion itself.

The most popular cause of malocclusion and defective speech is a kid’s constantly open mouth. This causes the tongue to take a bad position in the mouth and the dental arches do not develop as they should. And once they fail to develop, they cannot hold all teeth properly. As a result, teeth crowding follows, which can pose a real problem when permanent teeth erupt as they simply do not have enough space to grow.

Unfortunately, almost 80% of 4-5 year-olds keep their mouth open during the day and do not breathe through the nose only. In the environment of my 6-year-old son I frequently observe children who do not have the correct position of the tongue and mouth, which is very easy to notice since their lips are constantly parted and changes in facial features are already visible.

Parents, who can monitor their child closely, should respond to this problem and make sure that their child keeps its mouth closed, provided, of course, that the nose is not blocked. To cure the problem, ENT, speech therapy, myotherapy and orthodontic care may be necessary.

Open mouth can be caused by frequent respiratory infections. Pre-school kids spending most part of each day in the group of other children are very vulnerable to infections and can easily catch a cold and a runny nose. Therefore, also due to the fact that chronic upper respiratory tract infections can lead to habitual breathing through the mouth, nasal patency should be maintained and helped by keeping a child at home and strengthening its immunity.

A child with airway obstruction often can have an enlarged third tonsil and snore during sleep, which may worsen the malocclusion and speech defect even further. The more enlarged third tonsil, the more infection prone a child becomes and thus we end up with the vicious circle situation.

I would like to emphasize that also newborns or infants should sleep with their mouth closed. It is not normal for an infant to sleep with its lips ajar. If we notice that the baby has a tendency to lie with its mouth open, we should try to gently close it each time after feeding.

At this point I would like to mention that breastfeeding can be conducive to instilling the correct breathing path as a child must learn to eat, swallow and breathe with closed mouth at the same time.

Another cause of malocclusion and speech problems may be the incorrect positioning of the tongue in the mouth. The tongue should lie on the palate, behind the upper teeth and exert a slight pressure on it. If you see a person with a wide upper dental arch, straight teeth and a nicely shaped middle face (which is made of jaw bone) – that is the example of what it should be.

If a child has an open mouth, the tongue automatically falls towards the bottom of the mouth, thereby fixing the bad posture of the mouth. Incorrect position of the tongue will inevitably result in defective function, i.e. incorrect pronunciation and swallowing.

The key to success is not EXCLUSIVELY a therapy conducted once a week, but active exercises performed EVERY DAY at home under the guidance and with encouragement of the parents.

What is the interdisciplinary treatment?

More and more often speech therapists direct the youngest (aged between 2 and 3) preschoolers to an orthodontist because they detect a developing malocclusion. Such a child is too young to be treated with an orthodontic appliance. However, it should be kept under control of an orthodontist whose role is to make the parents aware of related problems, strengthen their motivation to work with their child (exercises, check ups or consultations at other specialists) and finally initiate an active treatment when a little patient is ready for cooperation (taking impressions and wearing an applaince is impossible without it). It usually happens around the age of four or five.

Treatment of malocclusion in young children should be interdisciplinary. First of all, we should focus on preventive action and strengthening the correct position of the mouth – it is a crucial starting point for any further actions. So I will repeat again: corrective exercises must be performed EVERY DAY! Thanks to this, a well-positioned tongue, a natural orthodontic appliance for every human, will affect the development of dental arches with all teeth well fitted in the future. A child’s face will grow in the forward direction (which is attractive) instead of vertical (unattractive longitudinal shape).

At the same time, if there is a need (and for most children this is the case), the expansion of dental arches should be started with an orthodontic applaince. At the age of 5-10, approximately 99% of the facial skeleton development is completed. The mandible grows a bit later, adjusting itself to the maxilla (?), which is the middle part of the face. So how do you influence the forward jaw growth if it’s already completed by the age of 10? Unfortunately, children with jaw hypoplasia are still very frequently sent to orthodontic treatment only after all permanent teeth are erupted… Well, we can align the teeth of a teenager, but unfortunately at this point we have very little influence on their facial growth.

In summary, the orthodontic treatment of preschoolers should be as complex and comprehensive as possible and focus on solving problems in various areas. The awareness and motivation of parents is extremely important because it is primarily up to them to direct their child’s development.
